PROGRAM DETAILS

A healthy baby starts with a healthy pregnancy. Any Baby Can is a proud provider of Nurse-Family Partnership (NFP), an evidence-based, home-visitation program that aims to improve pregnancy outcomes, decrease pre-term births, and empower mothers and their partners to create better futures for themselves and their babies.

Beginning in early pregnancy, first-time moms are matched with a registered nurse to bring answers, guidance and confidence to this transitional stage in life, all in the comfort of their home. The nurse is available to the family until the child turns two years old. NFP does not replace other medical appointments, but rather supplements those visits with personalized, whole-family support.

Eligibility Criteria

  • First-time, pregnant moms
  • Before 28 weeks of pregnancy (the earlier the better!)
  • Resident of Travis or Williamson County
  • Under 185th Federal Poverty Level (qualify for WIC/Medicaid)

Services

  • In-home or virtual nurse visits through pregnancy and post-partum
  • Health screenings and monitoring
  • Registered nurses specialized in maternal and infant care
  • Certified lactation consultants
  • Maternal mental health services
  • Guidance for high-risk pregnancies, pre-eclampsia, NICU stays, and more
  • Access to community resources for women and children, including doctors, insurance, childcare and job training
  • Continuity of care by collaborating with physicians, midwives and nurse practitioners, and encouraging clients to attend prenatal and well-child appointments

WHAT IS A VISIT LIKE?

Our registered nurses meet with moms in their homes about once each month. Visits give the pregnant or new mother a chance to discuss health, family dynamics, community resources and child development.

Maybe you’ve always wanted kids, or maybe you were thrown into parenthood. Our nurses can help. Each visit starts with a check-in for both mom and baby. Parents can ask questions, receive health screenings, and establish personal and family goals.

As part of our visits, we do health screenings and check mom's blood pressure. Preeclampsia is one of the main causes of mortality for pregnant women, so our registered nurses work with clients to understand the warning signs and how to have a healthy pregnancy.
